We can only accept new patients who live within our practice area (within 4 miles of the surgery).
Please use the practice boundary map below to check if you fall within this.
- If you are outside our area, you can visit the NHS Choices website to find your nearest GP surgery.
- If your address falls within our practice boundaries, please follow the link below to submit a registration form.
- Ensure you include your contact details—you will then receive a text message requesting some additional information.
Once completed, please note processing takes 7–10 working days, as we request your health records from your previous GP.
Once you submit your details, A member of our reception will verify your address. We may ask you to provide proof of your address in the form of Photo ID or a Utility bill.

Your Named GP
Upon registration, you will be assigned to Abbots Bromley Surgery Pooled list (under the care of all our GP’s) However, you may request appointments with your preferred GP (subject to availability).

Non-urgent advice: Notice
Your registration pack will include a detailed New Patient Questionnaire.
This enables us to gather information about your current and past health, which is very useful to us as we have to wait for your medical records to come from your previous doctor.
Temporary Residents
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
