If you’ve been off sick for more than 7 days
To request a sick note:
- Please call Reception, use the NHS App, or complete our online non urgent medical request form to submit a request.
When you contact reception to make this request, a member of our team will need to ask you some specific questions to allow the Clinician to process your request. You will also be asked whether you would like a paper copy or for your sick note to be texted to your mobile.
When submitting an online form your request will be forwarded directly to a doctor, if we require any further information, or the doctor wishes to see you before being able to issue your sick note a member of our team will contact you. Unless you state otherwise in your online request your sick note will be texted to you.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work.
