Patients should self certify for sick leave under 7 days and a form can be downloaded below. For sick leave 7 or more days patients should book an appointment.
Less than 7 days Self Certification
If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self-Certification form yourself.
However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £20.
More than 7 days Sick / Fit Notes
If you have been ill for more than 7 days, please book an appointment to speak to a doctor. If you have have been in hospital or had an operation you can try and request using our Online Admin Request link:

- The doctor may decline your request and you will be notified to book a GP appointment
- The practice aims to process requests within 5 working days
- Please be reassured that a note can always be back dated if required
