New Patient Registration

Catchment Area

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (PRF1) and a health questionnaire which will provide useful information.

The Nurse will then call you in for a pre-registaration interview. The rules and regulations of the practice will be explained to you and you will have the opportunity to ask any questions to the Nurse.

Once your application has been accepted, you are then registered with the surgery and are booked in for a new patient health check, whilst we wait for your medical records to arrive from your previous doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.

Please bring your medical card to reception. If you are unable to find it, you are still able to register. You will be given a form to complete – please do so as fully as possible.

Medical treatment is available from the date of registration. Please contact reception for further information.
