Self-Certification
If you have fallen ill and will be off for seven days or less, you do not need a sick note. However, your employer may request you to complete a self-certification form. This form can be obtained from either your employer or from this HMRC page here.
Requesting your first sick note
If your illness remains after the self-certification period, you can request a sick note from the GP. You would need to have a telephone consultation.
Repeat Fit Note Requests
Please allow up-to 48-hours between requesting and collecting the repeat fit-note
Repeat fit-notes/ sick-notes can now be requested by using the links provided above. Unless you would like to speak to a GP, you do not need to book a follow-up telephone consultation. You can have the sick note sent to you via text or you can collect it at the surgery.
