If you’ve been off sick for more than 7 days
To request a sick note:
- If you need a sick note, you can contact the surgery. The best way is to use Patchs – our online booking platform. If you are unable to access Patchs please put in a Fit Note Form
- Alternatively, you can contact the surgery on 0121 796 2777 or visit reception in person to request a sick note.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Extension of an existing sick note
If you have already had a sick note for your illness, a GP may not need to see you again to issue an additional sick note.
