New Patient Registration

We welcome all new patients to GPS Healthcare.

Anyone in England can register with a GP surgery.

You do not need proof of address or immigration status, ID or an NHS number.

Medical treatment is available from the date of registration. 

You will need to complete a registration form below, which will provide useful information whilst we wait for your medical records to arrive from your previous doctor, this can take some time.  The questionnaires can be found in the links below.

It would help if you could provide proof of identity, for more information see below “examples of photo ID and proof of address”. Please bring this to your first appointment for us to verify. 

If completing the form is difficult for any reason, please speak to reception who will put you in touch with our Health and Wellbeing team to support you with the process. 

Once you have registered, you can contact us online or speak to reception if you have repeat medication that needs adding and issuing. 

Whilst awaiting registration please order enough supply of your repeat medication from your previous practice. 

If in the interim you need an emergency prescription visit 111/Emergency Prescriptions

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Please note we aim to process all new registrations within 2 weeks.

If you are unable to complete the form online you can fill out this paper form: Patient Registration Form and bring it into the surgery.  This will take longer to process so we advise that you register online.

Warning advice: What should I do if I have repeat medications?

When registering with us we would like to ask patients to obtain a full supply of repeats from their current GP before submitting the form. This is to ensure you have an adequate supply before being registered and your information being transferred over.

Could we also ask that patients submit a photo/scan of their paper repeat slip or screenshot of full repeat medication list from the NHS App with their registration – this will also signal to reception when registering to add these to the notes.
