Improving Support for Learning Disabilities

One of our Care Coordinators supports our team to ensure more learning disabled people are going to the doctors to have the Annual Health Checks they are entitled to. This is the next step in helping to reduce health inequalities in our area and make our people and communities fairer, stronger and more connected.

Acute Care Team

The Acute Care Team is a multi-disciplinary team made up of GPs, Advanced Nurse Practitioners, Paramedics and an Acute Care Team Care Coordinator. This team offers on the day appointments to patients between 8-6.30pm and are  based at our Village surgery site.

You can and will be offered a telephone or face to face appointment dependant on your medical problem and we can accommodate video consultations where requested.

We will always do our best to accommodate your preferred option. 

Improving Cancer Care

Anyone registered with our practice, upon diagnosis of a cancer, will be offered the opportunity to speak to one of our GPs and our Cancer Care Coordinator. This can be really helpful in gaining extra support at a difficult time, as well as help and expertise about and social, emotional or financial support that you maybe entitled to.

Our Health and Wellbeing team will remind you if you are eligible for a Cancer Screening program, will discuss the programs with you if you are unsure what it involves or it worries you in any way.

Social Prescribing

Social Prescribing links patients to services. These services can help with financial services, care packages, befriending and even community groups. They have particular expertise in helping those over the age of 75 access support and services that maybe of benefit. 

Mental health: Did you know there’s more than one service which can support your mental health that can be accessed today and doesn’t require a referral from a GP?

Bereavement: Grief is different for everyone, but you don’t need to do it alone. Even just sharing fond memories can help you grieve. We can also support you finding a way to grieve that you feel most comfortable with.

Isolation: Not sure of what groups are up and running again? Or looking for something to do or someone to talk to? We know many a place!
Carers Support: We can get your started on a carers package, refer you to specialist teams and create a safer environment for your and the person you care for to help ease the stress of being an informal carer.

Healthy Lifestyle: We can give you access to cheaper classes through Solihull active, help motivate you to stay on track, and talk to you about diet, in a way that works around your life

Group Consultations

The aim of this project is to connect and educate patients to enable them to support themselves and each other at the same time as having longer with the GP or specialist nurse.

The project involves seeing patients in group settings, and encouraging them to communicate and learn with one another, to think of how to achieve their goals. They will spend time being supported in this both by a group facilitator and a doctor, nurse or other professional. These will initially be done remotely using the internet and are very new to the practices but have been widely studied and used around the world for years.

We have offered Group Consultations in Diabetes and Menopause care.
