Residential areas served by the practice include Palfrey, Caldmore, Pleck, and Delves. When registering, please bring a Proof of I.D including a Utility Bill or Bank Statement, and Proof of Address. You will need your NHS number (from your previous GP), and complete a registration form and new patient questionnaire. If you move out of the practice area, you will need to register with a new practice.
We are located at 151 Wednesbury Road, Walsall, WS1 4JQ, and serve the area within the practice boundary. Please call our reception staff if you are interested in registering as a new patient and want to know if you live in the practice boundary.
Online Services Information – Proxy Access
If you wish to, you can now use the internet to:- book appointments with a GP- request repeat prescriptions for any medications you take regularly- look at your medical record onlineProxy user access can also be set up for children, relatives and dependants. This enables a parent, family member or carer to act on your behalf with their online access tailored accordingly.You can also still use the telephone or call in to the surgery for any of these services as well. It’s your choice.Being able to see your record online might help you to manage your medical conditions. It also means that you can even access it from anywhere in the world should you require medical treatment on holiday. If you decide not to join or wish to withdraw, this is your choice and practice staff will continue to treat you in the same way as before. This decision will not affect the quality of your care.You will be given login details, so you will need to think of a password which is unique to you. This will ensure that only you are able to access your record –family members or carers can be provided with their own login details for access to your patient record, this is known as ‘proxy access’.The practice has the right to remove online access to services for anyone that doesn’t use them responsibly.It will be your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately.If you can’t do this for some reason, we recommend that you contact the practice so that they can remove online access until you are able to reset your password.If you print out any information from your record, it is also your responsibility to keep this secure. If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all.

You can register by completing a registration form which can be downloaded below or obtained from the practice. Please bring the completed form along to reception with your medical record if you have it.
When you register, we will ask you to make an appointment to see the healthcare assistant for a new patient health check. This will make sure that our records are accurate and all routine checks, to promote your health and well-being, are up-to-date. Please ensure that you attend such appointments.
The practice is required to allocate all new and existing registered patients a named and accountable GP at Palfrey Health Centre.
Individual patients will be informed of their named accountable GP at the earliest available opportunity.
All patients over 75 years of age have been nominated Dr S Siddiq as their GP.
All patients under 75 years of age have been nominated Dr A Ahmed as their GP.
Where patient’s express a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.
Having a named GP does not prevent you from seeing any other doctor at the practice. Your named GP will not be available at all times, and if your needs are urgent, you may instead discuss them with an alternate GP.
