Practice Boundary Area

Warning advice: Important Patient Notice

Our practice is within the West Cheshire Clinical Commissioning Group Area. We can only look after patients who live within our current catchment area. Our map below shows our current boundary area. We can only register patients living in England. Please be aware that our catchment area does not cover patients who live in Wales as they fall under a different Clinical Commissioning Group Area, you will need to find a GP in Wales. If you move over the border to Wales you will be asked to register elsewhere. Please check the boundary map before registering on-line to avoid issues. If unsure, please ring us to check on 01244 385553 before registering on-line. Children cannot be registered without a parent already registered with us or registering at same time.

Catchment Area

We are using a new online service called “Register with a GP surgery” that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register. Paper forms are still available if you need one. Contact our reception team here: 01244 385553.

IMPORTANT INFORMATION – BEFORE REGISTERING ON-LINE – PLEASE ENSURE YOU HAVE ENOUGH MEDICATION TO SEE YOU THROUGH FOR THIS MONTH IDEALLY AS IT TAKES TIME TO GET YOUR FULL MEDICAL RECORD ON OUR SYSTEM AND REGISTRATION FORM PROCESSED WITH OUR RECEPTION TEAM. YOU CANNOT RING UP AN APPT AS SOON AS YOU HAVE COMPLETED THIS FORM ON-LINE. THIS TAKES TIME TO BE PROCESSED WITHIN OUR SYSTEM AND HAVE HIGH VOLUMES OF PATIENTS REGISTERING EVERY WEEK. IT CAN TAKE UP TO TWO WEEKS TO GET YOU FULLY REGISTERED AND RECORDS RECEIVED. BEAR THIS IN MIND BEFORE LEAVING YOUR CURRENT SURGERY. CHECK YOU HAVE ADEQUATE MEDICATION.

The surgery is currently accepting new patients.  We require you to complete the official GMS1 purple registration form (both front and back) plus the Patient Questionnaire (see below). You can download both and print off at home if your prefer or we can post one out to you: 

It normally takes up to 48 business hours for your application to be processed on to our system but can be up to two weeks dependent on volume received and staff levels. Ensure you have enough medication to keep you going for a good few weeks before registering with us as mentioned above.

Students

We welcome student patients at the practice.  Please pop into the surgery to complete an application form.

Visitors / Temporary Residents

If you are visiting the area or are a temporary resident, we may be able to see you as a temporary resident.  Please contact the surgery for further information.

If you feel that you require immediate medical attention, there is an Urgent Care centre based at the Countess of Chester Hospital.

What Happens Next

Once your application has been accepted by the practice, we will apply for your medical records from your previous surgery.  Please note that this can take up to 6 weeks as per NHS guidelines.

Please ensure that you have enough medication from your previous surgery to last whilst we transition you over to our practice.

Should you need medication whilst your records are being forwarded to us, please make a routine appointment with a GP.
