We are excited to share an incredible opportunity to join the Central and West Warrington PCN team as a Care Coordinator. This role is perfect for someone passionate about patient care and supporting individuals to navigate the healthcare system effectively.
About the Role
As a Care Coordinator, you will:
- Work alongside a network of 7 GP practices in Warrington.
- Support patients with proactive care, including booking screenings, chronic disease monitoring, and vaccinations.
- Help patients transition between services and access the care they need.
- Be a key point of contact for patients, families, and carers.
This diverse and fulfilling role focuses on improving healthcare access and outcomes for the local community.

Qualifications & Skills

- Knowledge of NHS systems is beneficial but not essential as full training will be provided.
- Ability to work independently and as part of a team.
