Annual Review Invites

As part of our ongoing Process Improvement initiatives in the Surgery we will be moving annual chronic condition review invites to a single birthday month invite. This means that instead of you receiving a few invites per year you will just receive the one and have the opportunity to have all conditions reviewed in the one appointment.

These appointments will cover the following reviews:

• Diabetes
• Hypertension
• Chronic Heart Disease
• Stroke / TIA
• Asthma
• COPD
• Bronchiectasis
• Thyroid conditions

We plan to roll this out from November this year.  For the first year you may receive an invite less than 12 months after your previous review so this will be the reason.
