How to Make an Appointment
Online Consultation
We are now offering online consultations, submit your request below. Use this form to request an appointment with a member of our team (GP, Nurse, Physio, Mental Health, Phlebotomy, Health Improvement Practitioner, Pharmacist, Paramedic, Advanced Clinical Pharmacist), order medications, request repeat sick notes, admin queries for our secretarial and admin team (including enquires regarding hospital appointments, referral queries, private letters and reports, hospital correspondence) . We will respond to you within 48 hours.
NHS App
You need to prove who you are to get full access to the NHS App. With full access you can:
- order repeat prescriptions and nominate a pharmacy where you would like to collect them
- book and manage appointments
- view your GP health record to see information like your allergies and medicines (if your GP has given you access to your detailed medical record, you can also see information like test results)
- book and manage coronavirus (COVID-19) vaccinations
- get your NHS COVID Pass (there are also other ways to get you covid pass)
- register your organ donation decision
- Receive notifications and messages from practice and national
- Book 1st hospital appointments using your UBRN number
- Use the Symptom Checker
- choose how the NHS uses your data
- view your NHS number (find out what your NHS number is)
- use NHS 111 online to answer questions and get instant advice or medical help near you

My GP App
You can download the app on www.mygp,io/app and book an appointment for the clinician of your choice.
Appointments can also be made by contacting reception on 01204 661600. The times of our surgeries can be found under ‘opening times’.
Please note that our Saturday morning slots are for pre-booked nurse appointments only and not for emergency issues.
Urgent Appointments
If you require a urgent on the day appointment then please contact reception on 01204 661600, you may not be able to see your usual Doctor but we will endeavour to ensure you see a clinician on the same day to access urgent consultation.
Appointments Evenings & Weekends
We can also offer a variety of appointments (GP, ANP, Practice Nurse, MSK and Mental Health Practitioner) weekday evening 6.30 pm to 9.30 pm and Saturday’s. These appointments are held at Southbrook Surgery, Southbrook Grove, Bolton, BL3 2DN.
Cancellations
If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

Think Pharmacy First
It takes five years for pharmacists to become fully trained.
So they are experts in the use of medicines and are qualified to diagnose and treat minor illnesses.
For many illnesses, your local pharmacist can provide free advice and medication to help you and your family feel better. Pharmacists are also required to continue to update their knowledge and skills at all times.
How can Think Pharmacy First help you?
We know that people visit their GP with minor illnesses that their local pharmacist could help with. Instead of booking a GP appointment, you can visit your local pharmacy instead, saving you time and with no appointment needed, you can just walk in.
All pharmacists can give advice and medicine if you need it that will help clear up the problem.
If your condition is more serious they will then refer you to your GP.

Can I get free treatment?
If you don’t normally pay for your medicines, then you don’t need to pay with Think Pharmacy First – you can get them free and over the counter from your local pharmacist without the need to see your GP.
Home visits
If possible please try to telephone reception before 10:00 if you require a home visit and let us know if your condition is urgent.
When you request a visit the receptionist will need to ask you full details of the patient’s name, age, address and telephone number and the reason for the visit. This information is essential for the doctors to plan calls and deal with emergencies promptly.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.
If you are expecting a home visit please could you place any pets in a secure place, away from where the doctor or nurse will be examining you.
You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Late for your appointment
Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

We have a texting service which allows you to receive confirmation and reminders about your appointments.
To have this service you will need to register by completing a consent form.
