Tracy has worked within the NHS for over 22 years at Swan Lane Medical Centre. Tracy has had a variety of roles within the practice during her 22 years + employment with Swan Lane Medical Centre including medical receptionist, PA to Senior GP, Medical Secretary & finance, wages & pensions administrator and Assistant Practice Manager. She took over as Practice Manager on 1st April 2019 and manages the business aspects of the practice.

Anne has been a member of the Swan Lane Medical Centre practice team for 30 years working as IM&T Manager for many years. She took over the role of Assistant Practice Manager from 1st April 2019 working along side the Practice Manager.
Reception
Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.

We have a team of social prescribers talk to patients about what matters to them, and what support they need to feel happier and healthier. They know what is available in the community, so connect people to local groups, activities and services that make a lasting difference.

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
