Registration
If you wish to register with this practice, please call into the practice for a registration form or complete the pre-registration form on the website. If you can, please bring some identification and proof of address. We will also need you to complete a brief medical history form to allow us to treat you safely whilst we wait for your medical records to arrive at our practice.
From then on, you will be registered with this practice and your medical records can than be requested from you previous doctor.
Because this can take some time, you will be offered a new patient medical with one of our Practice Nurses. She will take details of relevant medical history that your new doctor will need to be aware of any repeat medication you are taking.
Please note that you will not be able to request repeat medication until you have seen either the nurse or the doctor.
You will also be given a practice leaflet, which has our telephone numbers and details of the surgery times and clinics at the surgery
Medical treatment is available from the date of registration. Please contact reception for further information.

Online ‘Pre-Registration’ With The Practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. We also need you to bring along 2 forms of identification before we can register you. One needs to be photo id and the other confirming your address.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you to the practice.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the area where you are already registered.

GP2GP
The practice is now using an electronic facility which can allow the medical record of patients registering with this practice to be transferred safely and quickly from their previous GP. Similarly, if you register with another GP, your medical record would be transferred electronically to your new GP. At the moment, this can only happen between GP Practices that are using this facility.
This will not affect how you register with the practice but means that if your previous GP also used GP2GP, your consultations and medications will be available for your new GP to access from the date your register with our practice. Your paper medical record folder will still be sent to your new Doctor as it is now.
