Patient Forum

Dear Patient,

We would like to know how we can improve our service to you and how you perceive our surgery and staff. 

To help us with this, we have a  Patients’ Forum so that you can have your say. We will ask the Forum some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

In addition, we meet face-to-face at the Health Centre two or three times each year.  These meetings bring the group together with practice staff, and usually include a presentation on a topic of current interest.  Meeting times are varied through the year to enable a diverse membership.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.  If you are happy to join us, or for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

Our Patient Forum is encouraging patients to give their views about how the practice is doing.  We would like to be able to ask the opinions of as many patients as possible and are asking if patients would like to provide their email adresses so that we can contact you by email every now and then to ask you a question or two.
