Booking an Appointment
Please complete our Total Triage form Request appointment online – a doctor will look at your form the very same day, and we will let you know what comes next as soon as possible, by phone call or text message. Our forms will open at 8am every working day.
Please provide as much information as possible on the form:
- What your query is about
- What action you would like
- Your preference for who or when you would like to deal with your problem.
- You will be invited to submit photos if relevant.
If you are unable to complete the form, our Patient Service Advisors will be able to help you – call or visit reception at either site.
Your appointment
However you choose to contact us, we may offer you a consultation:
- In person at Dickenson Road Medical Centre, or at a nearby surgery
- by telephone
- by video call
- by text or email
Appointments by telephone, video call, by text or email can be more flexible and often mean you may get help sooner.
If you need help with your appointment
Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
Practice Nurses
In a number of cases it might be worth considering an appointment with a practice nurse rather than a doctor.
Practice nurses are qualified to deal with many conditions and you may be seen more quickly.
Languages Spoken
English, Hindi, Urdu, Punjabi, Gujarati, Kiswahili and other regional languages of the sub-continent of India.
For any other languages a translator can be organised if we are given at least 4-5 days’ notice; a telephone interpretation service is also available.
Hospital Appointments
Should the doctor think that you need to go to hospital he/she will discuss this with you and explain why.
The doctor will request an appointment using the Gateway/ICH service.
If you are required to telephone or contact the hospital to complete your referral you will receive a letter from them (Integrated Care Gateway) telling you to do so.

Home Visits
If possible please try to telephone reception before 11:00 if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit; there are also better facilities for examining and treating patients at the health centre.
