Access to Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Such requests should be made through the practice manager and may be subject to an administration charge. No information will be released without the patient consent unless we are legally obliged to do so.
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.
Most complaints can be resolved in practice. Please speak to a member of the team if you have a concern and they will assist you where er possible. Alternatively, if you with to speak to the Practice Manager – Claire Hollinshead you can request to do so. Please note an appointment may need to be booked if she is not available at the current time.
If you do not wish to speak to the practice staff you can request that the Integrated Care Board (ICB) investigate your complaint, they will then contact on your behalf:

Complaints should be within 12 months of the occurrence, or 12 months from when you become aware of the matter. We will aim to respond within three business days. We will investigate and provide the findings as soon as possible and will provide updates regarding the investigation.
We will ensure all complaints are investigated with the upmost confidentiality and are not held within the patient health care record.
One a final response has been issued with the full details and the outcome of the complaint we will notify you.
Confidentiality & Medical Records
The practice complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:
- To provide further medical treatment for you e.g. from district nurses and hospital services.
- To help you get other services e.g. from the social work department. This requires your consent.
- When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us know.
Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
Freedom of Information
Information about the General Practitioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager.
GP Net Earnings
Average earnings calculation for the year ended 31 March 2019
All GP practices are required to declare the mean earnings for GPs working to deliver NHS services to patients at each practice.
The mean earnings for GPs working in the Chowdhury Practice in the financial year ended 31st march 2019 was £77,000 before deduction of employee`s superannuation contributions, tax and National Insurance. This average pay for the 1 full-time GP and 3 GP locums who worked in the Practice for more than 6 months during that year.
Safeguarding At Chowdhury Practice
Safeguarding in the practice is seen as a service that all staff are involved in; Safeguarding issues can be raised by all team members from the reception team to a GP.
Safeguarding is also the responsibility of all patients, if you have any concerns regarding any patients, family members friends or neighbours. YOU can refer into the local safeguarding teams, here’s how:
What to do if you have concerns about individual children or adults
Report abuse or neglect of an adult.
If you believe you are, or suspect another person is, in immediate risk of harm call 999 and speak to the Police.
All calls concerning worries about abuse and neglect of children or adults will be treated seriously.
Don’t ignore the abuse or neglect. Don’t keep it secret or put off reporting it. You should act even if you are not 100% sure that abuse is taking place.

Violence Policy
The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.
