1. Our champions will ensure our patients receive the following to ensure great care:
2. Ensuring that the quality of Healthcare the Practice provides to patients with Learning Disabilities is reviewed.
3. Ensure that Pathways relating to Learning Disability Issues are implemented within the Practice.
4. Promote Learning Disability Equality within the Practice.
5. Be involved with the implementation of the Audit Action Plan within the practice.
6. Be the named contact within the Practice for patients with Learning Disabilities and their carers. 
7. Attend Training and Forums relating to Learning Disability.
8. liaise & engage with the Primary Care Access Team on matters that require advice, guidance and support for Patients with Learning Disabilities.
