Social Prescribing

Our social prescribing link workers can help with a whole range of non-clinical needs that can have a profound effect on your health and wellbeing. They have time to listen and work with patients over a number of weeks both face to face and by phone/video.

They can put you in touch with community support and groups, as well as come along with you to get you started. They can help you plan what you want to do and get there.

Although they’re unable to do it for you, they’ll be there for you the entire way, listening to your needs and providing all of the information that organisations and groups in SWAN can do to help you.

First Contact Physiotherapists

First Contact Physiotherapists

Our first contact physiotherapists are here to assess and diagnose any new aches, pains and injuries to your bones, joints and muscles. 

They offer expert advice on how best to manage your condition, arrange follow-up appointments and can refer you to a specialist for further treatment if necessary. They can also speak to the GP straightaway if you need any medication during your appointment.

Physiotherapists train for longer than GPs on issues with bones, joints and muscles so are the perfect specialists to see for these matters. Having a physiotherapist directly at your GP practice means you get seen quicker and recover faster. 

Clinical Pharmacists

Clinical Pharmacists

Clinical pharmacists are experts in medicines and how they are used. We are very fortunate to have clinical pharmacists in our team as they can help our doctors and nurses to spend more time with patients while they deal with the majority of medicine-related queries.

Our clinical pharmacists deal with repeat prescriptions, reviews, common minor ailments, urgent medication requests and deal with medication for patients recently discharged from hospital.

Our GPs may ask the clinical pharmacist to get in touch with you if you take lots of medications from time to time. This is to review your symptoms and check they’re still the best medications to be on. Where better, newer alternatives are available, they’ll discuss this with you and your GP.

Paramedics

Paramedics

Our paramedics usually see patients who cannot get into the practice or who require an urgent home visit.

Trained in nursing and paramedic skills, they are qualified to deal with everyday health concerns from coughs and minor injuries to more serious conditions such as asthma or heart attacks.

During home visits, the paramedic can access your full GP record and advise if further treatment or specialist care from the hospital, is required. They report directly back to the GP with the outcome of the visit and any updates on any treatment and medication that was given.

As well as home visits, they help our practice teams manage urgent appointments as well as telephone triage. By doing so, they free up time for GPs to handle more complex medical cases in the practice and offer urgent advice over the phone for patients unsure of whether they need to go to A&E.

Can’t make daytime appointments?

Patients at our practices are able to book evening and weekend appointments.

If you are registered with one of our practices, you can book appointments on evenings and Saturdays.

GP practices in SWAN work together to provide these extra appointments, increasing convenience for local patients and reducing pressures on other NHS services such as A&E. Patients can book to see a GP, nurse or other healthcare professional.

Services available include GP appointments, blood tests, medication reviews, coil fittings, vaccinations, immunisations and cervical smears. More services will be added as we continue to develop.
