New Patients

We can only accept new patients who live in our practice area. If unsure, please contact the reception.

To register at either practice you will need to attend in-person to complete a registration form and a new patient questionnaire. Please bring with you a proof of ID (driving licence, passport) and proof of your address (utility bill, bank statement). When you come to the surgery you will be asked to sign these forms to confirm that the details are correct.

Failure to bring the above will result in not being able to be registered at the time.

In order to speed up the registration process, you may print the registration and medical questionnaire form for each person below, complete and bring them with you to the registration appointment.

Parents may register children under 16.

Adults will be required to book a new patient health check appointment with the nurse and a urine sample will be required (pots are available at reception if required).

Non- English Speakers
