What is a Primary Care Network?

Primary Care Networks were introduced in 2019 as part of the NHS Long Term plan.  As the population grows and people live longer, often with more complex needs,  GP services have had to adapt to these changing needs.

GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in local areas in groups of practices known as Primary Care Networks.

Additional Roles Reimbursement Scheme (ARRS)

Primary Care Networks have been given funding to provide additional roles to create bespoke multi-disciplinary teams within their individual network. 

PCN’s assess the needs of their local population and, working with local community services, make support available to people where it is most needed.

Who we are

Primary Care Networks (PCNs) were established in July 2019 as part of the NHS Long Term Plan to improve the quality and accessibility of care by encouraging collaboration among various healthcare services and community organisations. Our PCN, one of 13 PCNs in Somerset, is made up of five GP Practices; Vine Surgery (Street), Glastonbury Health Centre, Glastonbury Surgery, Wells City Practice and Wells Health Centre. How to access our services

We accept referrals from various sources to ensure our services can be accessed by all. You may be referred to us by your GP practice, or from other organisations such as the local council or community groups. Additionally, you can contact us directly about any health and wellbeing concerns that you have, to see how we are able to help.

What we do

Our aim is to create an integrated and sustainable healthcare environment that prioritises a personalised care approach for the people of West Mendip.

Those registered at any of the GP Practices within our Primary Care Network (PCN) have access to our full range of services, offered across the entire network. Residents can still access GP appointments through their GP Practice.

To book a GP appointment, please contact the GP practice you are registered with. In addition to this, we can provide guidance and support on a wide range of health and social care topics, from preventive measures to managing chronic conditions.

We may also refer our service users to non-clinical services in the community, addressing social, emotional, and practical needs to improve overall health and wellbeing.
