What is a Primary Care Network?

The new GP contract was published in early 2019 as part of the NHS Long Term Plan. A key element of the new contract is the development of Primary Care Networks (PCNs) which bring GP practices together with other local services – such as community, mental health, social care and the voluntary sector  to look after local populations.  PCNs help to join up services at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.

Our surgery teams are working closely with each other, enjoying the ability to share expertise and resources, to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the need of our patient population.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.

Our PCN Team

DR BEENA GOHIL – CO-CLINICAL DIRECTOR

I graduated from Kings College London in 1995, and after a number of years in hospital jobs in general surgery, orthopaedics, general medicine, anaesthetics, intensive care and gynaecology, I qualified as a GP in 2002.

I have worked at Oldfield Family Practice since 2002, and as co-clinical director of Greenwell PCN since 2019.

I have enjoyed the challenges offered by the clinical director role, and we have formed good relationships across the 7 practices, and expanded our team with the 7 roles we have recruited. The clinical director role has allowed me to form good working relationships with Ealing CCG, neighbouring networks and Ealing GP federation. We have worked collaboratively with other networks, and were involved with the delivery of covid vaccinations at Ealing Town Hall between January and May. I look forward to expanding our team at Greenwell, and continuing to deliver excellent care to patients registered across our PCN.

DR VIC SIVANESAN – CO-CLINICAL DIRECTOR

I graduated from St George’s Hospital, University of London in 2004, and spent the next 6 years in a number of hospital posts including general medicine, general surgery, cardiology, respiratory, intensive care, A&E as well as paediatrics, obstetrics & gynaecology and palliative care medicine. I qualified as a GP in 2011 after spending my training year in a local Greenford GP surgery.

I have worked at Mansell Road Practice since 2013, and as co-Clinical Director of Greenwell PCN since 2019.

I have continued to provide general practice care to my patients as well as developed into the role of Co-CD with Dr Gohil. We have good relationships across the neighbouring practices, and have recruited 7 roles into the network and aim to continue to add to our team. We have worked well with other networks, the CCG and Ealing GP Federation and I look forward to working with our partners and patients to make positive changes and additions to our network.

LUKE HEDLEY – FIRST CONTACT PHYSIOTHERAPIST

Hi. My name is Luke Hedley, I graduated from Hertfordshire University with a first class honours in 2008. Prior to this I also completed a degree in Sports Rehabilitation at St Marys University College. I have since worked in both the NHS and several private clinics gaining a broad depth of experience in musculoskeletal Physiotherapy.

First Contact Physiotherapy is a service that aims to manage any of the Greenwell PCN patients who have musculoskeletal pain. It is predominantly an assessment and triage role whereby, following an in depth assessment, we can discuss ways to help you manage your pain, or arrange referral to other services/specialisms, should they be required.

Currently, the service I offer means that you will receive a telephone or video call first to conduct an assessment. Following this, if further information is needed to plan your management, I will ask you to come in for a face to face appointment at either Mansell Road Practice or Hanwell Health Centre.

IFEOLUWA POPOOLA – FIRST CONTACT PHYSIOTHERAPIST

Hi, I am Ifeoluwa Popoola, a First Contact Physiotherapist (FCP) working with Greenwell PCN, a graduate of the University of Ibadan, Nigeria. I have deployed over 17yrs of my professional and clinical expertise both in Nigeria and Uk in meeting expectations and goals especially in Musculoskeletal Physiotherapy (soft tissue and sporting injuries, Arthritis/joint pain, spinal pain including lower back, mid-back and neck pain).  

I gladly welcome you to any of the Greenwell PCN  GP surgeries to experience bespoke consultations that helps relieve your pain and restore function. Thank you.

SAJNEE SHAH – PHARMACIST

Hello, my name is Sajnee Shah and I am one of the PCN pharmacists for Greenwell.  I have been qualified since 2015, my background has primarily been in hospital.  I started this role in July 2021 and I’m currently supporting the 7 practices with tasks such as medication queries, signposting/referrals, drug monitoring, medication safety alerts, meeting local and national standards, and medication reviews. As this role continues to develop, I hope to improve the health outcomes for all the community.

Hi my name is Priyanka. I have recently joined the PCN as a pharmacist. I have been qualified for 5 years now and have always worked in community pharmacy until I moved here.

ADINA MIHALE – SOCIAL PRESCRIBER

Hi, my name is Adina Mihale. As Social Prescriber for the PCN I offer non-medical services that aims to help people improve both their health and wellbeing.

FAHAD ALI – SOCIAL PRESCRIBER

Hi, my name is Fahad Ali and I am a Social Prescriber within Greenwell PCN. I started working as a Social Prescriber in September 2021 and my role involves working with people to address their unmet social needs and connect them with agencies and community services in order to improve their health and wellbeing. I am passionate about addressing the wider determinants of health in order to improve the populations wellbeing and health outcomes.

Hi, as well as sharing the administration duties of the PCN with Gurvinder, I am also the Practice Manager at The Mansell Road Practice.

VICTORIA HAMERTON – ADMINISTRATION

Hi, my name is Victoria Hamerton. I am the IT Administrator for the PCN, helping with any issues relating to the clinical IT system the practices use. I have worked in General Practice since 1998, working with some of the Greenwell PCN practices individually since 2015. I am also the IT Lead/Assistant Practice Manager for another Ealing Practice as well as a paediatric phlebotomist.
