We facilitate the delivery of monthly diabetes/lipids MDT. This involves GPs and other clinicians bringing exceptional cases to the meeting to discuss/formulate the best care for those patients. Each month consultant from Chelsea and Westminster Hospital will join to aid the GPs/clinicians.
MDT discussion of complex needs of children/families within the primary care network, the meeting is followed by face to face consultations with the families to carry out action which may come out of the meetings.
Thus far we have only facilitated VGCs for diabetes interventions, we invite patients across our PCN to engage with a GP via teams, the hope is that once patients discuss their difficulties and successes the overall awareness and management of the primary care networks diabetes population increases; leading to better health outcomes.
