How we can help.
- The service provides personalised care and support and improves access to local support services to help people with cancer to lead as full and active life as possible
- Some examples of how they can support: Holistic Needs Assessment, Personalised care planning and goal setting management
Extended Access Service
We know it can be difficult for some people to get to our practices during the week and we have a range of appointments available with experienced, trained professionals in the evenings and at weekends including:
- GP: face to face and telephone appointments (Sat /Sun)
- Nurse: providing face to face appointments e.g. for long term condition reviews, smears (Sat /Sun)
- Healthcare Assistant: providing face to face appointments e.g. bloods, health checks (Sat /Sun)
- Long-acting Reversible Contraception (LARC) clinics (Sun)
- Pharmacist: providing telephone appointments e.g. medication reviews
- Physiotherapist (first contact) (Mon-Fri)
- Healthy Minds (support with anxiety, low mood etc) (Mon-Fri)
Our face to face appointments are in 2 of our practices – New Cross and Gibson Lane

In the UK, an average of 30% of GP appointments are for musculoskeletal (MSK) problems. Having physiotherapists work within GP practices and take over these appointments, means that patients get seen quicker and recover faster.
Plus – the workload is reduced for GPs helping to ease pressure off general practice.
First Contact Practitioner (FCP) is a new model evolving UK. It involves placing physiotherapists directly into GP practices and to treat patients who come into the clinic with musculoskeletal problems.

Health Coaching:
- Enables you to identify lifestyle changes and develop an action plan to improve your health outcomes
- Empowers you to take responsibility for your health & improve your self-management of your Long-Term Condition
- Encourages you to develop your coping strategies

Linking Leads – Social Prescribing
Social prescribing can link you with services within the community to support you to improve your health & wellbeing. They can help with:
- Housing or money issues
- Wellbeing & emotional support – links with community groups
- Loneliness
- Meaningful social activities
What we do
We can link patients with services and activities within their community to improve their social, economic and mental wellbeing by;
- Giving patients an opportunity to discuss what is important to them.
- Link you patients with appropriate services within their community to support them to achieve their goals.
- Provide patients with their own wellbeing coordinator who will help to plan their next steps.
Please contact Linking Leeds to discuss any potential referrals. All referrals will be allocated to the wellbeing coordinator’s working within the relevant PCN.
