Within this PCN, a care coordinators primary focus is to support patients in a variety of creative ways through their health care journey.
The role is both clinical and admin based, supporting all members of the PCN staff with different projects and work streams involving patients. Their role is to put the patient at the centre of any care provided. They work in many creative ways to ensure the patient journey and access to any of our services is appropriate, meaningful and accessible.
For example, they have worked on projects such as the Autism project, working in collaboration with Autism charities to ensure patients are able to access the practice, through advocating for the patient and ensuring implementation of reasonable adjustments.
They are involved in all projects across the PCN.
Care coordinators are particularly good at supporting patients with complex needs and difficulty managing their own health care journey. They complete a What Matters to Me assessment with every patient to help keep the patients priorities at the centre of all care provided.
