
LSMP and TLS PCN Pharmacy consist of 4 clinical pharmacists and a pharmacy technician.
Our pharmacist are the medicines experts and how medicines effect the body and when they are used for different medical conditions.
The pharmacy team play a pivotal part in the NHS’s investment and impact fund and the quality improvement schemes within Leeds.
The pharmacists and pharmacy technician work within the General Practice teams, consulting and treating patients directly. They can be the first port of call for patients with medication queries and reviews. The pharmacists provide extra help to manage severely mentally ill patient’s medication.
Medicines related policies are aligned across both sites where possible.
Structured medication reviews are untaken by the pharmacists to ensure the risk of harm to patients are reduced, this is shared decisions making and creating individualised plans for the patient.
The clinical pharmacist offers both telephone and face to face consultations. The consultations may include physical examinations such as blood pressure measurements, heart, temperature, respiratory examination etc.
Our pharmacy technicians ensures patients are making the most of their medicines by considering the safety and cost effectivity.
