The PCN has 2 Community Wellbeing Connectors (CWC) which are a key part of a new model within Leeds, helping bridge gaps between people and support in their communities.
The purpose is to support people to achieve better mental health and wellbeing, increasing independence and supporting recovery.
The CWCs support patients with ongoing, complex and/or Severe Mental Illness (SMI).
They support people to consider and navigate the different types of community support available to them, also offering practical support, i.e., accompany to activities/assessments if needed.
We aim to make initial contact with people within 48 hours of referral and arrange an assessment within 10 working days. The support period is currently undefined and assessed on an individual need basis.
The CWC service is managed by Northpoint
