Health and Wellbeing Coach

Health and Wellbeing coaches support people with complex and multiple long-term conditions. They focus on personalised health and care plans with smart goals. They help patients make more informed and conscious choices. Enabling people to develop the knowledge, skills and confidence to take opportunities.

Health and Wellbeing coaching sessions involve a structured conversation with a coach, you will set 1 – 3 goals each session. The first appointment is up to an hour to get to know you, the following sessions are around 30 – 45 minutes. Your sessions are booked weekly or fortnightly, depending on your preference and availability.

In your sessions we will explore:
– Increasing autonomy and motivation
– Providing supportive challenge
– Exploring your current situation and options of change, including risks and benefits 
– Managing stress levels and executive function.
– Supporting self-reflection to understand the relationships and connections between emotions, feelings, and behaviours.
– Explore and challenging health beliefs, where appropriate
– Increasing motivation, knowledge, skills, and confidence to enable people to self-manage, where appropriate

Most appointments are offered as face to face although if you cannot make a face-to-face appointment, you can change your appointment to a telephone call if needed. It is recommended that your initial appointment is face-to-face as this will help provide you with the best support.

You will be given your own health and wellbeing booklet to record your goals as well a place to record and review strategies and motivation levels

The coach can set up an automated reminder service for you for appointment reminders if you find this would helpful.
