Registration With The Practice
You will be required to fill in a registration form and produce proof of address (i.e. utility bill, bank statement) and photographic evidence (i.e. current passport, driving licence). (If you cannot provide these two forms of ID, then please ring the practice to discuss your registration)
As part of our registration process, you will be offered a health check with the nurse or health care assistant to discuss current health status, concerns and medication.
Please complete the form below – you can download the form, complete it, and then bring it to the surgery with two forms of ID as explained above. You can also collect this form from our reception.
