Booking non-urgent appointments online
We have an online system for booking non-urgent appointments or placing requests such as fit notes, results, doctor’s letters, or referrals.
This allows us to make sure you seeing the right person as quickly to meet your needs. Click the link below and fill out the form here: https://florey.accurx.com/p/Y02847
Answer all the questions carefully, including times when you are not available. We try to offer appointments that work for you, but sometimes it’s not always possible. If you can’t make your appointment, please fill out the form again on a day you can.
A doctor will look at all appointment requests.
We like to contact you by text message to tell you what will happen next. Please keep your phone nearby and watch for our message. For example, we might offer you a phone call during a two hour window or ask you to come to the surgery for a face-to-face appointment.
If the doctor calls, they will try twice to reach you. If they can’t, they will ask you to fill out the form again the next day.
When all the GP appointments for the day are full, the online request form will be turned off. It will be available again the next morning at 8am.
