When asked, please supply a working email address and contact number along with details of your symptoms.

Once you submit your information one of the following 3 things will happen;

  • You will be able to book an appointment of your choosing online
  • Or an appointment booking link will be sent to you by email following a review by a clinician
  • OR we may call you on your supplied contact number should we require further information.

Please contact the surgery to inform the practice staff in the following circumstances;

  • If you have any disability, impairment or sensory loss that means you are unable to book online
  • Require an interpreter
  • Require a face to face appointment but are unable to attend the surgery in person due to your health
  • Require any assistance to book an appointment or make an admin request

If you have multiple problems to discuss that are not relevant to each other, please complete a triage form per problem.

Test results – if you need a follow up, we will contact you directly. If you wish to check your results you can do so online via the NHS App.

Admin Queries – this can be used to request continued sick notes, request letters or reports, referrals queries, repeat prescriptions and update your personal details.

If you need urgent medical advice between the hours of 6.30 pm – 8 am weekdays, during weekends or on Bank Holidays – please call 111.

NHS 111 can help if you have an urgent medical problem and you’re not sure what to do.

In case of a life-threatening emergency, please dial 999.

Rushcliffe patients can access routine appointments in the evenings and at weekends.
