Most Patients choose to register with a GP that’s closest to them, we do cover most of Brent and in some instances, we are able to register patients out of the area (restrictions may apply).

Out-of-Area Registration: What You Need to Know

Did you know you can register with us even if you don’t live within our catchment area?
Under the NHS Out-of-Area Registration scheme, we can accept patients who live outside our geographical boundary.

However, it’s important to understand what this means:

  • What’s included: You will still have full access to our in-practice services, including appointments, prescriptions, and general medical care.
  • What’s not included: Certain services, such as home visits, may not be available to you. Instead, you may need to contact NHS services in your local area for assistance in these cases.

  • Bring them to our reception desk in person during our opening hours.

  1. GMS1 Registration Form
  2.  The Health Questionnaire – We need to know as much health information about you as possible, to allow us to give you the best possible care.

If any of the persons being registered are under 5 years of age we need to have all their immunization history.
