Accountable Named GP for All Patients
The practice is required by the Government under the terms of the latest GP contract to allocate all patients a named accountable GP.
For convenience if you have a medical card your accountable GP will be the named doctor you are registered with. Patients registering over the last 2 to 3 years will not have received a medical card as these are no longer routinely sent when registering with a new GP.
However, the practice does keep a record of your registered (accountable) GP. If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
Please note that there is no need to telephone the practice for this information.
Where a patient expresses a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.
Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
Chaperones
A Chaperone Policy is clearly displayed in the waiting area, reception and in all of the clinical consultation rooms. All staff who Chaperone are DBS (Criminal Records) checked.
Complaints
If you have a complaint about the surgery or the services we offer, please let us know.

Confidentiality
If you wish to speak to the receptionist in confidence, there is a quiet area available away from the main reception.

General Data Protection Regulation (“GDPR”)
The GDPR requires practices to process data fairly and in a transparent manner which is easily accessible and easy to understand. This means that practices must provide information to patients about how the practice processes patient data in the form of a ‘practice privacy notice’.

GP Average Earnings
However it should be noted that the prescribed method for calculating earnings is potentially misleading because it takes no account of how much time doctors spend working in the practice, and should not be used to form any judgement about GP earnings, nor to make any comparison with any other practice.
My Care Record
My Care Record enables health and care professionals to access information about you to improve your care. The Hamilton Practice is part of My Care Record, an approach to improving care by joining up health and care information. Wherever possible, health and care professionals will be able to access your records from other services when it is needed for your care. For example, a hospital doctor, community nurse, or social worker involved in your care could view the information they need from your GP record. NHS Integrated Care Board
An NHS Integrated Care Board (ICB) is an organisation with responsibility for NHS functions and budgets.
NHS Summary Care Records
As part of a mandatory, national programme each GP Practice will have to make a summary care record for each patient (unless the patient has already opted out). Please read the information regarding this.

Planning and Research Data
We are providing data to NHS Digital as part of the General Practice Data for Planning and Research data collection, so that patient data can be used to help improve everyone’s health and care. If you would like to opt out please download and complete a Register your Type 1 opt out preference form (Microsoft Word) and return it to your surgery.
Safeguarding Adults and Children
The aims of the practice are to ensure that we:
- Stop abuse
- Prevent harm and reduce the risk of abuse
- Safeguard adults in a way that supports them to have choice and control
- Promotes an approach that improves the lives of the adults concerned
- Raise public awareness so that communities play their part in preventing abuse
- Provide information and support in accessible ways
- Address the cause of the abuse or neglect
All of our staff have had a DBS (Criminal Records) check and receive the relevant Safeguarding Training.

Your Health Records
To help your GP provide you with even better care The Hamilton Practice, along with others in west Essex, is participating in a scheme that allows your GP health record to be linked with records other health professionals have about you. If you wish to opt out of your pseudonymised data being extracted please write to our Practice Manager and your records will be coded to avoid any extraction being made. Zero Tolerance to Violence Policy
All practices, in line with government guidelines, have a ‘Zero Tolerance to Violence’ policy. This means that any violent or abusive behaviour or perceived threatening behaviour, whether verbal or otherwise to staff or members of the public on practice premises will not be tolerated. We can refuse to provide a service, report the incident to the Police and request that the patient and their family be removed from their Practice list.
