Total Triage
On October 1, 2025, many GP surgeries in England implemented a new “Total Triage” system where all requests for GP or urgent appointments must first be submitted via a simple online form. This form, available on the practice website, the NHS App, or a smartphone, allows patients to describe their symptoms or needs, which are then assessed by a senior clinician to determine the best next step or signpost them to other services.
Total triage means that every patient contacting the practice first provides some information on the reasons for contacting the practice; and is triaged before making an appointment.
The main purpose of the Total Triage model is to: enhance the quality of care our patients receive. Ensure that patients are given the most appropriate appointment or advice to meet their needs. The practice will communicate with our patients in a timely manner.
All medical appointments will be triaged by our medical receptionist.
It will be faster for you to submit a triage request online where you will be contacted by our team with an appropriate appointment.

National Data Opt-Out
The National Data Opt-out lets you choose whether your confidential patient information is used for research and planning beyond your individual care. It does not affect the care you receive.
How to set or change your choice
- NHS App: Open the NHS App → Your Health → Choose if data from your health records is shared for research and planning.
- Online: Visit Your NHS Data Matters (manage your choice).
- Phone: Call 0300 303 5678 (free from most mobiles/landlines).
- Post/email: Paper forms are available via the NHS website (Other ways to make a choice).
What’s covered
- Applies to confidential patient information used for research and planning (secondary uses).
- Does not apply to information used for your direct care, or to anonymised data.
Our commitment
