To request a fit note or an extension to a fit note please click on the button below which will take you to an online request form. Select the fit note option.

If you have already had a Fit Note for this illness your Doctor may not need to see you to issue an additional Fit Note

Less than 7 Days sick

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. We call this self-certification and you would normally refer to the policies for your place of employment.
