End of life care

End of life care is support for people who are in the last months or years of their life.

End of life care should help you to live as well as possible until you die and to die with dignity. We aim to ask you about your wishes and preferences and take these into account we they work with you to plan your care.

We work with our specialist Community Palliative Care team, our District Nursing Team   and St Michaels hospice team to deliver your care.

If you are a person with a terminal condition or you are caring for someone who is terminal, then St Michaels Hospice have a 24 hour helpline, which you can call to talk through what you need and how that can be provided.

With our specialist palliative care team, and our district nurses, we will look after people when they are dying at home
