Connect to your GP services online and save time. Quick, easy and secure.

Online Access and Records

You may be able to use online services to:

  • Book, check or cancel appointments with a GP or nurse
  • Order repeat prescriptions
  • See parts of your health record, including information about medicines, vaccinations and test results
  • See communications between your GP surgery and other services, such as hospitals

How can I do this?

If you are a newly registered patient, online access to appointment booking and ordering repeat prescriptions can occur after your first appointment with a GP to discuss your medications (this is a safety review). You must complete a New Patient Questionnaire (Form A) and a Online Records Access Consent Form (Form B) for this.

Information about full ‘prospective’ records access

Although you can book appointments and order medication online immediately after registering with us, there may be a delay for FULL online records access. This is because it is not safe for everyone (eg if history of domestic abuse and open to coercion) therefore we need to wait for your full medical record to reach us and be summarised and coded. This can take up to six months.

If you have been registered for more than 6 months and do not have access, you can fill in and submit Form B, or send us a message requesting access. We will reply with a text message questionnaire to ensure you understand the implications of access, and will then check your record to ensure it is appropriate for you. This may take a few weeks.


There are some things to consider before you create an account. Please read the information below carefully:

Although the chances of any of these things happening are very small, you will be asked that you have read and understood the following when you sign up for online services.

Proxy Access
If you are using this service to access another person’s account as a proxy (eg as a carer or parent), you MUST have discussed appropriately what you are accessing and for what purpose. Generally we do not award proxy access to parents/carers of children aged over 11.

When a child reaches 11 years of age, parental proxy access will automatically be withdrawn.

Between 13 and 16 years, a child may be able to consent to online records access by a parent carer.

After 16 years, it is assumed a child has capacity to consent to access by a parent or carer.

This service will be reviewed at regular intervals by the GP practice and may be rescinded at any time.

The proxy access request form can be accessed from our medical secretaries.

All requests will be reviewed by the clinical team and we will send you a questionnaire to read and complete. Then your records will be reviewed to ensure safety. You will be informed of the outcome.

NHS App

Try the NHS App. If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. You can use the NHS App to get health advice, order repeat prescriptions, manage your hospital referrals, view your GP health record and more. If you already use Patient Access you can continue to use it but you can use the NHS App as well. For more information go to www.nhs.uk/nhsapp.

What the NHS App does

  • get advice about coronavirus – get information about coronavirus and find out what to do if you think you have it.
  • order repeat prescriptions – see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to.
  • book appointments – search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments.
  • check your symptoms – search trusted NHS information and advice on hundreds of conditions and treatments, and get instant advice or medical help near you.
  • register your organ donation decision – choose to donate some or all of your organs and check your registered decision.
  • find out how the NHS uses your data – choose if data from your health records is shared for research and planning.
  • view parts of your GP record – including information about medication, allergies, vaccinations, previous illnesses and test results.

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time.   • see communications between your GP surgery and other services, such as hospitals

How to register for Online Services

If you have not used our online services before you will need to:

  • Contact the surgery by phone or in person
  • Verify your identity
  • Secure a username and password
  • Enter Patient Access and enter your registration details exactly as shown on the form.
  • After registering, log on with your username and password.
