New Patients

Kingstone Surgery welcomes new patients who live within our practice area. Please make sure all members of your household are registered, not just those who currently use GP services.

Registration can be achieved on completion of the appropriate forms, which are available from Reception. These consist of:

  • 1. NHS registration form (GSM1 form) – click here to download (soon these may be able to be done online)
  • 2. New Patient Health Questionnaire (Form A) – click here to download (this is for safety and health information)
  • 3. Online Records access consent form (Form B) – click here to download (this can also be done by text message)

You may be asked to produce evidence of address e.g. household bill. We are unable to register patients (including temporary residents) beyond our practice area. Again, unfortunately, due to capacity, if you move outside of our practice area we will unable to continue to be your medical practitioners.

We still need you to come in to the surgery for a signature from the registering patient where appropriate.

Registering with the practice

When contacting the surgery for the first time please mention that you have completed these forms. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct – please note we are only able to register you once you have signed the forms. 

It can take a considerable time for us to receive your medical records, and we ask you to book a first GP appointment to meet a clinician and we can record your allergies and medications, and make sure your monitoring is up to date.

Online services at registration

We activate limited online services via the NHS app for ALL patients on registration to enable ordering of repeat prescriptions and booking appointments online.

However, we cannot activate full online records until we have received your full medical record from your previous GP practice, and this has been fully coded. This may take up to six months.

If you want full online records access we will ask you some questions to make sure you are fully aware of the implications of this (Form B). Alternatively, contact us online to request a text message so you can fill in the questionnaire this way.
