Our Pharmacy Practitioners are employed by the Practice and work within Marton Medical Practice premises at Whitegate Health Centre. They are NOT attached to the chemist shop within our building.
Their role is to support the practice and our Doctors with Medicines Management
They are available to help our patients with a range of issues including the management of a range of long-term health problems, patients on complex regimes or large numbers of different treatments including pain management.
They are happy to support patients needing advice or having problems such as side effects with their medicine.
Reviewing medication for our patients, including patients with complex regimes is an important part of their role as Medicines Management Pharmacists.

Are our Advanced Clinical Practitioners. They have undertaken specialist training to Masters degree level and can see patients with undiagnosed problems, they can perform physical examinations, make a diagnosis, treat and refer patients in a similar way to a GP.
They work with patients to develop strategies to improve their health and prevent illness with health education and counselling. The Nurse Practitioner works closely with the Doctors and other members of the nursing team.
Community Midwives
During Pregnancy it is important for expectant mothers to be seen regularly to ensure that mother and baby are progressing well. Our Community Midwife, Pauline, runs our weekly antenatal clinic on Wednesday mornings. She sees expectant mothers in clinic by appointment and visits.
Community Nurses
The District Nursing Team is based in Whitegate Health Centre. They aim to provide high quality, effective nursing care for patients in the community and offer specialist wound care; Palliative Care for those who are terminally ill, and support for family and carers; advice and support for people with chronic illnesses; nutritional care and advice; access to specialist equipment; instruction on self care to enable patients to manage their own care. The practice receptionists forward all messages for the team.
The Community Matron and her Team are based in Whitegate Health Centre and provide specialist nursing care for patients with Chronic Conditions, in their own home. The practice receptionists forward all messages for the team.
Health Visitors
The Health Visitors are fully qualified nurses and Community Specialist Practitioners in Public Health. They provide advice on health care, particularly for expectant mothers and pre-school children. They are based in Whitegate Health Centre and you can speak to them by telephoning 953025 or 953026.

Sandra Crossley is our Specialist Diabetic Nurse. She manages all aspects of Diabetes care, including insulin management, for our patients, with assistance from our Health Care Assistants who carry out the physical aspects of the Diabetic check up.
Practice Nursing Team
Our Well trained and highly experienced Registered Nurses (RN) can help you with all aspects of Health Promotion, Management of Chronic Conditions such as Asthma, Diabetes and Heart disease etc. Travel Vaccinations and health advice for travel, Blood Pressure Checks, Ear Syringing, Blood Tests. They also carry out Cervical Smear Tests, HRT checks and family planning checks and injections.
Our very experienced Nurse Associate and Health Care Assistant work under supervision of our Registered nurses and carry out health checks, health promotion, blood tests and blood pressure checks along with a range of other nursing tasks including flu, pneumonia and shingles vaccinations and vitamin B12 injections.

The nursing associate is a generic nursing role in England that bridges the gap between healthcare support workers and registered nurses, to deliver hands-on, person-centred care as part of a multidisciplinary team in a range of different health and social care settings. The nursing associate is a relatively new role within primary care which provides nursing support to the practice.
Nursing associates are hands-on and person-centred professionals who work with registered nurses and healthcare support staff to deliver care, freeing up registered nurses to focus on more complex care.
Nursing associates work across adult, children’s, mental health, and learning disability fields of care.
A nursing associate will;
support newly diagnosed patients and their families with care and information, perform and record clinical observations (blood pressure, temperature etc.) display skills such as cervical cytology, immunisations and vaccinations (under direction of a prescribing clinical professional) and wound care. undertake clinical tasks such as venepuncture and ECGs
Healthcare Assistants (HCAs) in General Practice work under the guidance of registered healthcare professionals to provide patient care. In the surgery, they are important patient-facing roles that have both clinical and administrative responsibilities. They may collect vital signs such as pulse, temperature, blood pressure and blood glucose levels. The HCA may also be asked to set up equipment, update patient health records and ensure that medical supplies are in stock.
Types of duties may include;

Care Navigation and Triage Hub Manager
Liza
Liza supervises the Care Navigation team, manages patient appointments and resources, and ensures efficient patient flow and communication between patients and staff. The role involves leadership, training, and support for reception staff, as well as managing the operational effectiveness of the front desk and supporting the Practice Manager.
Liza also manages the administrative side of our Practice Triage Hub and our team of Hub staff, who work together with the Duty doctor to manage and process all incoming patient requests and queries
Care Navigation Team
Hub Team staff, are our senior care navigators. They work together with the Duty doctor to manage and process all incoming patient requests and queries, are trained to help patients find the most appropriate service first, not always the GP, by directing them to the right professional like a pharmacist, social prescriber, or nurse, ensuring faster, more suitable care for their needs, improving access, and reducing unnecessary GP visits. They act as a crucial communication hub, guiding people through complex health and social care systems.

General Care Navigators are the First Point of Contact for patients with the Practice. They asks questions (as directed by our doctors) to understand the patient’s issue and urgency. They also deal with general queries and back office tasks and functions such as processing prescription requests, clinical coding, contacting patients as directed by clinicians etc.
