Registering with the surgery
To register with Walkden Medical Centre:
- Fill out an online registration form
- Visit the surgery after 10am to collect a paper registration form.
Once you have registered, you may be required to book a new patient health check so we have all your information. We’ll contact you to let you know if we do.
Find your NHS number
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Named accountable GP
All our patients have an allocated named GP, which is your registered GP. If you wish to know who this is, please ask at reception.
Please note that you can still choose to see the GP of your choice.
