The Rumworth PCN has a patient population of 33,000 and comprises four member practices:

• Garnet Fold Practice
• Swan Lane Medical Centre
• Dr Mirza & Partners
• Shanti Medical Centre

The PCN is supported by the Bolton GP Federation, which has employed a dedicated team, including a Network Manager and a Deputy Network Manager. The team works alongside the Clinical Director to manage the project specifications, including Directed Enhanced Services (DES) such as Early Cancer Diagnosis, support for care homes, and Medicine Optimisation, among others.
