The Hammersmith & Fulham PCN (Central) was formed in April 2019 with five like-minded practices wanting to provide services to our local communities.
Since the NHS was created in 1948, the population has grown, and people are living longer. Many people are living with long term health conditions such as Diabetes and Heart Disease and may suffer with Mental Health Issues. We aim to provide easier access to our services to help you with these conditions.

Our aim is to provide more integrated services for all our patients and improve access to them, connecting Primary Care to our local communities.
