Restoring Previous Access
Patients who previously used the NHS App should continue to do so. If it has not fully relinked to the Practice, please uninstall and reinstall the App. The option to relink should appear when you try to access your health record or request a prescription.
Patients who only used Patient Services previously will need to register for the NHS App (preferred) or Patient Access.
If your NHS App is connected, but your access to records has not been fully restored, please be patient. It will take some time to process all the account requests. You do not need to complete a new form unless we ask you to.
If you need urgent access, are missing your linkage key or have any questions about online services that are not answered on these pages or the dedicated support pages, please email stw.dmp.online@nhs.net. You must provide your name, date of birth, NHS number and Post Code if you require a new linkage key. The email must also come from your registered email address. We reserve the right to request further identifiable information if needed.
If you did not have access to your records on the NHS App with our clinical old system. You will need to complete the access request form. Downloadable below or available from reception.

We as a practice do not have administration rights over your online accounts or apps. If you are having any issues at any stage of setting up or accessing the NHS App or Patient Services. Please contact the relevant support first. They will then advise if you need to speak to us or will contact us directly.
NHS App
More recently, the official NHS App has been available for patients to register with. This application allows for registration without having to fill in forms and is very quick.

Patient Access
Some of our patients use Patient Access to manage appointments, order prescriptions and view sections of their medical records.
You can access quick support via this link: the material ranges from helping you reset your password (if you’ve forgotten it or are locked out of your account) to initial registration and includes everything in between (e.g., how to order prescriptions).
