As a practice we are constantly looking at new ways to deliver the best care to all of our patients. As part of this, we are excited to announce our new appointment system, Rapid Health, which will be launched on 8th December 2025.

The easiest way to book an appointment is to use the NHS APP:

Urgent appointments

To request an urgent appointment for today or tomorrow (Monday to Friday):

  • use our appointment request form, Monday to Friday from 8.00am to 6.30pm. We will respond by 6.30pm the same day
  • phone us on 01204 848411, Monday to Friday from 8.00am to 6.30pm
  • visit the surgery and speak with a receptionist, Monday to Friday from 8.00am to 6.30pm

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Routine appointments

To request a routine appointment:

  • use our appointment request form, Monday to Friday from 7:00am to 6:30pm. We will respond within 1 working day
  • phone us on 01204 848411 Monday to Friday from 8:00am to 6:30pm
  • visit the surgery and speak with a receptionist, Monday to Friday from 8:00am to 6:30pm
  • use your NHS account (through the NHS website or NHS App) to book a screening test or vaccination

When you get in touch, we’ll ask what you need help with.

Non-urgent advice: Help for patients who can’t access the form

For those patients who do not have the facility or ability to access us digitally, our team of experienced receptionists will support them to do so, offering education on how to access the system and assistance. Moving forwards we encourage those who can use eConsult to do so.

If you need an interpreter or if you have any other access or communication needs please call the practice on 01204 848411 and one of our team of experienced receptionists will help you

Contact Your GP Online

How do I use Rapid Health?

To access our services such as appointment, prescription requests, and administrative queries such as fit note requests or chasing referrals you will need to complete an online form. Rapid Health is an NHS approved online system that makes booking an appointment easier and faster and saves you time telephoning the surgery.

To do this…

  • Click on this link 
  • Select either admin or medical request
  • Type in your main concern or symptom
  • Choose the right question
  • Enter your details
  • Answer the questions by selecting the option/s that relates to your problem/concern
  • Confirm this is a non-urgent request
  • If you need urgent help you will be told what to do
  • You will be offered an appointment
  • Book your appointment. Select your prefered date and time to be seen and Health Professional if offered:

    Where – click on the magnifying glass
    Who with – click on the magnifying glass
    Choose the date you prefer and click on the word ‘show’
    Click on the time you prefer
  • Then click ‘confirm’ and you will get a confirmation email
  • That’s it……..all booked
  • If you are not offered an appointment one of our Health Professionals will review your request and if its urgent will get in touch before 6.30pm the same day either by email, text or phone. All other requests you can expect a response and advice from the practice by 5pm the following day.
  • Please do not submit more than one form for the same problem/concern

How to book an appointment if you receive an email with a link in it

  • click on the link
  • enter your date of birth
  • choose an appointment option by clicking on the arrow next to:
  • Where – click on the magnifying glass
  • Who with – click on the magnifying glass
  • Choose the date you prefer and click on the word ‘show’
  • Click on the time you prefer
  • then click ‘confirm’ and you will get a confirmation email
  • thats it……all booked

Your appointment

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Evening and Weekend Appointments

Evening and weekend appointments are available and are held at Heaton Medical Practice, 2 Lucy Street, Bolton, BL1 5PU.

You may be offered and be able to choose an appointment during these times when you have completed your appointment request form.

When you contact the practice state that you would like an evening or weekend appointment.

To cancel an evening and weekend appointment:

  • click on the CANCEL link in your appointment confirmation email
  • call the practice on 01204 848411 and your call will be diverted to the service
  • reply CANCEL to your appointment reminder text message

Home visits

If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.

To request a home visit, it is helpful if you phone the practice between 08:30am and 10:00am on 01204 848411  

Cancelling or changing an appointment

To cancel your appointment:

  • click on the CANCEL link in your appointment confirmation email
  • use your NHS App or your NHS account (through the NHS website)
  • reply CANCEL to your appointment email or reminder text message
  • phone us on 01204 848411 Monday to Friday from 8:00am to 6:30pm and choose the ‘Automated check and cancel service, option 1’
  • using our online form

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

If you need help when we are closed

If you require urgent medical assistance which cannot wait until the surgery re-opens, please call the Out of Hours service on 0161 763 8940

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
